Cosmetic surgical procedures can produce profound, long lasting changes in facial appearance, but are invasive and have an associated recovery period. Many patients who are just beginning to show signs of facial aging desire improvement, but cannot justify the cost or time required for surgical intervention. For these patients, the rapidly expanding repertoire of injectable treatments is perfect to suit their needs. Injectables can be used during an office visit rather than requiring a trip to the operating room. With some treatments, no anesthetic is necessary as the needles are so small that discomfort is minimal. With others, topical anesthetic agents or nerve blocks may be used, depending on the patient's preference and the sensitivity of the region being treated.
The cosmetic form of botulinum toxin, often referred to by its product name BOTOX® Cosmetic, is a popular non-surgical injection that temporarily reduces or eliminates frown lines, forehead creases, crows feet near the eyes and thick bands in the neck. The toxin blocks the nerve impulses, temporarily paralyzing the muscles that cause wrinkles while giving the skin a smoother, more refreshed appearance. Studies have also suggested that Botox is effective in relieving migraine headaches, excessive sweating and muscle spasms in the neck and eyes.
Soft Tissue FillersSoft tissue fillers are more useful for treatment of firmly established wrinkles or larger lines of facial aging (such as the nasolabial folds). Fillers restore volume to the face and can add structure as well. Depending on the type of filler and the depth at which it is injected, you can smooth out fine lines on the surface of the skin, fill out deep lines (eg: nasolabial folds), augment soft tissues (such as the lips), or even effectively augment facial bone structure. All of these injectable fillers are placed by injection, so the group carries usual risks of bruising, lumpiness, redness, product specific adverse reactions, and in rare cases local infections.
Juvederm© is a hyaluronic acid filler FDA approved to last up to one year. It is used to treat wrinkles and folds around the mouth known as “marionette lines” or “parenthesis.” Hyaluronic acid contains no animal proteins, and it is a naturally occurring substance in the human body. This lowers risk of allergic reactions to the injectable. Results from the treatment are immediately visible.
Restylane© is another common FDA-approved filler containing hyaluronic acid. It is used for wrinkles and folds around the outer corners of the mouth and can be used to add volume to lips. Results can be seen immediately after injection and lasts about six months.
Radiesse contains microspheres made of calcium hydroxylapatite suspended in a gel carrier. The filler is broken down by the body in two phases, first the carrier and then the spheres. During this process, collagen in-growth occurs, which is expected to provide a long lasting effect. This prolonged degradation process makes Radiesse a longer lasting filler than those previously discussed, with expected results lasting a year or more.
Sculptra® is an injectable poly-L-lactic acid designed for the restoration and/or correction of the signs of facial fat loss (lipoatrophy) associated with human immunodeficiency virus. Sculptra works by introducing a matrix on which the body is able to increase the volume and thickness of the skin. Several treatment sessions are required, but treatments normally last for more than three years. Cosmetic indications are soon to be released which will broaden the use of this filler.

Skin peeling involves application of a chemical solution to sun-damaged, unevenly pigmented, and finely wrinkled facial areas. The procedure is meant to diminish imperfections by peeling away the skin's top layers. Chemical peels vary according to their specific ingredients and their strength. Depth of peeling action may also depend on factors such as how long solutions remain on the skin and whether they are lightly applied or more heavily or vigorously applied.
Enhanced Jessner’s PeelThis advanced light-to-medium depth peel effectively exfoliates with blends of AHA’s and salicylic acid. It lightens the skin by inhibiting pigment production. Jessner’s peels reduce breakouts, soften fine lines, control oil and help repair the skin. There is light to moderate exfoliation resulting in a more even and smoother skin texture.
The trichloroacetic acid peel is available in differing strengths and can produce dramatic results. The downtime varies with the strength of the acid used. These peels are excellent for photo-damaged skin, fine lines and wrinkles, and for abnormal pigmentation. This is a deeper peel than a Jessner’s peel with more downtime, but with more dramatic results.
The vitalize peel is another choice for any skin type. The formulation of mild acids provides a complete, consistent, and safe peel to treat photo damage, fine lines and wrinkles. A more healthy and refreshed skin appearance is seen with mild to moderate exfoliation.
This is a modified TCA peel specifically formulated by Obagi. The penetration of peel into the papillary layer of the skin allows for the improvement of acne scarring, hyperpigmentation, and overall skin texture. Most patients have 4-5 days of downtime and recovery.
The ViPeel offers a comfortable option in chemical peels and is suitable for all skin types. It requires no skin preparation and very little downtime. The results are visible in a week.

The delivery of high intensity pulses of broadband light is a laser treatment used for skin rejuvenation. This popular procedure requires little to minimal down time because it is a non-ablative therapy, meaning it does not damage the surface of the skin. The intense pulsed light is delivered to the deeper parts of the skin and leaves the outer surface untouched. IPL is used to treat red vascular lesions, rosacea and sun-damaged skin on all areas of the body. Unlike many lasers that target a specific area, IPL is total photo- rejuvenation and it helps to smooth and even out the skin tone. It usually requires 2-4 treatments spaced 3-5 weeks apart. IPL can also be used in conjunction with ALA (a light sensitizing solution that is applied to the skin and activated by a particular wave length of light) to treat acne and/or actinic damage. This procedure is called Photo Dynamic Therapy (PDT).
Laser hair removal is an increasingly popular non-surgical cosmetic procedure. It is a non-invasive, convenient method to reduce hair growth.
Depending on the area being treated, the process may take anywhere from several minutes to several hours to complete. Wake Plastic Surgery offers a diode laser. It uses a beam of energy that passes through the skin and is attracted to the pigment of the hair. The energy passes down the hair shaft and is absorbed by hair follicles that are in the active growth phase, thereby disabling the hair follicle in the deeper layer of the skin. The hair follicles that are in dormant phase are not affected by the treatment. In most cases, minimal pain should be experienced and no anesthesia is required.

Spider veins (telangiectasias) are small purple to red blood vessels often visible on the leg, ankle or foot. Occasionally, spider veins will appear on the face. The cause is unknown, but heredity plays a strong role in the development of this condition. These should not be confused with varicose veins, which are larger, deeper veins that are usually lumpy and sometimes deeper purple in color. Varicose veins may sometimes cause leg discomfort, whereas spider veins are usually pain free. Spider veins are progressive, and usually increase in number and visibility with age.
The oldest and most common treatment for spider veins on the legs is sclerotherapy. This involves injection of a sclerosing agent into the veins, which irritates the vessels, causing them to scar and become less noticeable. Some surgeons use a concentrated (hypertonic) saline solution, while others use different sclerosing agents. Often, the veins do not actually go away, but become less visible because of the reaction around the walls of the vein. The injections may sting a bit, especially if saline is used, but the discomfort does not last long.
Laser treatment is another option for spider veins. This is often the preferred method of treatment in the facial area.
More than one treatment is often required, since all the veins will not respond to the first treatment. Also, since this is a progressive condition, the patient can expect new spider veins to occur over time, which may require treatment every few years to maintain the improvement.
